How do I attach a pulley to a threaded rod?

Some sort of bushing or adapter is clearly required, but I don't know if anybody sells this sort of thing commercially or not.

Use 2 nuts, preferably the self locking kind, and lock the pulley between them. Make sure that the pulley inside diameter matches the outside diameter of the threaded shaft.
